Provisional Ballot Signature Cures


I forgot to sign my Provisional Ballot or my signature does not match

If you received notification from the Lee County Elections Office that there was an issue with your signature on your Provisional Ballot, you will need to complete and return the affidavit below along with a copy of one form of identification to ensure your ballot will be counted.

The signed affidavit and attached copy of ID may be returned by mail, fax, email (as an attachment), or hand-delivered to any branch office location. 

ALL signature affidavits must be returned before 5 p.m. on the second day after the election.
