2024 Special District offices include:
Captiva Erosion Prevention District, Community Development Districts, Fort Myers Beach Public Library District, Fort Myers Beach Mosquito Control District, Independent Fire Control and Prevention Districts, Lee County Mosquito Control District, Lee Memorial Health System Board of Directors, and the Sanibel Public Library District.

Candidate Qualifying Forms
 

  • DSDE 9 Appointment of Campaign Treasurer and Designation of Campaign Depository Initial Filing - Required - only if the candidate will actively campaign
    Any special district candidate who intends to accept contributions or make expenditures in connection with his/her campaign must appoint a campaign treasurer, designate a campaign depository, and file periodic campaign treasurer’s reports with the Lee County Supervisor of Elections. Contributions and expenditures include any made by the candidate in connection with his/her campaign.
  • Campaign Financial Reporting System Affidavit Initial Filing - Required 
    The Lee County Supervisor of Elections provides its own (CFR) Campaign Financial Reporting System for the filing of required campaign finance reports. The Lee County (CFR) Campaign Financial Reporting System facilitates the electronic filing of required campaign finance reports in lieu of filing original and signed campaign finance reports by hand delivery or standard mail.  Upon filing Form DS-DE 9, a candidate and his/her campaign treasurer and deputy treasurer (if one is appointed) will be provided a User Identification Number, password, and PIN access to the CFR System for the filing of required campaign treasurer’s reports.
  • DSDE 104 Candidate Petition Form Optional 
    A candidate for a special district office may obtain the valid signatures of 25 registered electors of the district. Submit signatures to the Lee County Supervisor of Elections for validation. Upon certification, by the Supervisor of Elections, that the candidate has obtained the required number of valid signatures, the signatures shall take the place of the filing fee.  Signatures must be submitted on or before May 13, 2024.

  • Form 1 Statement of Financial Interest for 2023 Required - submit at time of qualifying
    Form 1 must be filed electronically with the Florida Commission on Ethics via the Electronic Disclosure Management System (EFDMS) at the following link:
    https://disclosure.floridaethics.gov/Account/Login
    A copy of the electronically filed Form 1 must be provided with your final qualifying documents.

     
  • DSDE 302NP Candidate Oath - Non-partisan Office Required - submit at time of qualifying - EXCEPT Lee Memorial Health System Candidates
    A special district candidate must file the correct Candidate Oath for the office sought. This form may only be submitted during the qualifying period. 

     
  • DSDE 302NP Modified Candidate Oath - Non-partisan Office Lee Memorial Health System Candidates ONLY! - Required - submit at time of qualifying
    A Health System candidate must file the correct Candidate Oath for the office of Lee Memorial Health System Board of Directors. This form may only be submitted during the qualifying period.
     
  • Canvassing Board Meeting and Logic and Accuracy Testing Schedules Required - submit at time of qualifying
     
  • Filing Fee Submit filing fee at time of qualifying 
    $25.00 filing fee. A special district candidate may pay the filing fee with a personal check, cash, money order, or campaign check (if a campaign account has been established at a financial institution). If paying by personal check, money order, or campaign check, make check payable to the Lee County Elections Office for the EXACT amount. A candidate who obtains the required number of valid candidate petition signatures, for the office sought, will not be required to pay a filing fee.
