I received an Omitted Signature Cure Affidavit for Vote-by-Mail Ballot

If you returned your vote-by-mail ballot to the Lee County Supervisor of Elections but did not sign the Voter’s Certificate on the back of the certificate envelope, failure to include a signature may result in your ballot not being counted.

Florida law allows you to cure a missing signature on the Voter’s Certificate by submitting to the Lee County Supervisor of Elections a completed and signed Omitted Signature Cure Affidavit for Vote-by-Mail Ballot (Form DS-DE 139), along with a copy of your identification by 5:00 p.m. on the day before the election.

Please follow the instructions on the form carefully, as failure to do so may cause your ballot not to count.
